To check the status of a previously submitted Open Enrollment Application, please log onto the Open Enrollment Application and click on the blue "status" button located on the top of the page.  Enter the same User ID and password as when you originally submitted your Open Enrollment application. 

 
The following information may be used to enroll your student:

   Complete Enrollment Forms Online


Parents will need to complete the enrollment forms online at http://enrollment.susd.org   as the first step in the enrollment process. The Centralized Enrollment Office has 5 computer stations available which parents can utilize. Once you complete the enrollment documents online, please bring the confirmation page to the Centralized Enrollment Office along with other required documentation. (The additional required documentation is listed below.) Enrollment needs to be finalized in person at the Centralized Enrollment Office.

Enrollment Information 


Age Limitations


Kindergarten: Any student enrolling in kindergarten must be five (5) years old on or before August 31 in the school year for which you are applying, per ARS 15-821.

Kindergarten Early Entrance: Students who turn five (5) years old between September 1 and December 31 may be assessed to determine readiness for early entrance into kindergarten in the school year for which you are applying. To request an assessment, parents must complete an Early Entrance Application. Please submit the application and the $60.00 assessment fee to Community Education at either the Early Childhood Cholla Campus (11130 E. Cholla), Early Childhood Oak Campus (7501 E. Oak Street) or the Education Center (3811 N. 44th Street, Phoenix). Assessment will take place only in March and June at the Early Childhood Cholla Campus.
 

For additional questions regarding early entrance testing, please contact Community Education at (480) 484-7900. If you have any questions after the assessment has been given, please call Dr. David McNeil at (480) 484-6142.


First Grade:
Any student enrolling in first grade must be six (6) years old on or before August 31 in the school year for which you are applying, per ARS 15-821.


Open Enrollment
Open Enrollment applications are due by January 15. Applications submitted after January 15 will be considered after those submitted by the deadline. The enrollment deadline for Open Enrollment students is March 31. 

After the January 15 Open Enrollment application deadline and the March 31 enrollment deadline, parents may still apply for Open Enrollment; however, acceptance of Open Enrollment students will occur based on space availability at the school, grade, and/or program at the time of application.



In addition to completing the enrollment process, you will need to bring the following documents with you:

Birth Certificate
Immunization Records
Proof of Residency (acceptable are rental agreement, most recent utility (gas, electric or water) bill.
Guardianship/Custody Documentation (if applicable)
Unofficial high school transcript or most recent report card (for students entering grades 9-12) and standardized test scores, if available.
Withdrawal Form (If previously enrolled in an Arizona school)

The Centralized Enrollment Office also manages the Open Enrollment process for both out of District and in District parents.
How to Open Enroll
Throughout the School Year
The Centralized Enrollment Office is open throughout the school year (Note: The CEO office will not be open during fall, winter or spring breaks)
Savings to the District
Centralized Enrollment Office eliminates the need to have front offices at schools open all summer, saving the District additional money in staffing and utility costs.
 
Change of Student Information (i.e., address, phone, emergency contacts)
Parents/guardians who need to change student information for currently enrolled students are able to make these changes at the school site.
All changes made throughout the year must be completed in person; data will not be changed via phone or fax.
Parents/guardians must provide identification; only authorized parents/guardians can make changes to student data.
Parents/guardians will need to provide appropriate documentation for the change requested, i.e., proof of residency documents.  Example: utility bill
Parents must sign and date the Student Profile Report which will be printed by the school site; their signature authorizes the changes.
